Work Environment\n

This role is primarily remote, allowing for flexibility in scheduling and work-life balance. Our team operates within a structured framework of Monday to Friday, from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client needs. This position requires the Social Worker to be adaptable and on-call as necessary, ensuring consistent care for our clients. Communication is key across our interdisciplinary teams, including nurses, caregivers, and healthcare professionals, to facilitate coordinated and effective service delivery.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, taking into account emotional, mental, and social factors that may affect their well-being.

• Develop individualized care plans in collaboration with clients and their families, tailoring strategies to meet their specific goals and preferences.

• Provide counseling and emotional support to seniors, guiding them through life transitions, and helping them cope with challenges such as isolation, grief, or health concerns.

• Advocate for clients’ rights, ensuring they have access to necessary community resources, services, and support networks.

• Monitor clients’ well-being through regular visits, observing their living conditions, providing safety assessments, and addressing any emerging issues.

• Facilitate open communication and mediate potential conflicts within families regarding the care of their elderly loved ones.

• Empower clients and families by educating them about available resources, support groups, and programs that promote social engagement.

• Keep accurate, timely documentation of client interactions, assessments, and progress notes, ensuring adherence to ethical and legal standards of practice.

• Stay updated on prevailing social work practices, trends, and regulations to ensure compliance and effective service delivery.

Patient Population / Client Focus\n

This position primarily serves seniors within our local community who may be grappling with a variety of issues such as social isolation, mental health challenges, and navigation of health-related complexities. You will work closely with clients and their families, ensuring they feel empowered and supported in their journey of aging.

Qualifications\n

• Master's degree in Social Work from a regionally accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW) in Texas.

Compensation and Benefits\n

The annual salary for this position ranges from $60,000 to $100,000, commensurate with experience and qualifications.
