As part of a hospice center, you will collaborate with a network of healthcare professionals, including nurses, psychologists, and caregivers, to ensure seamless access to services. Our outpatient mental health clinic is equipped with the latest technology, enabling efficient communication and delivery of services for our clients.

Key Responsibilities\n

• Conduct thorough evaluations and comprehensive psychosocial assessments for seniors to identify their unique needs.

• Develop and implement individualized care plans in collaboration with healthcare teams, focusing on interventions that promote mental well-being and resilience.

• Regularly monitor and document client progress, adjusting care plans as necessary to optimize patient outcomes.
\n
• Provide counseling and emotional support to seniors and their families, helping them navigate the challenges associated with aging and end-of-life issues.

• Facilitate family support sessions and workshops aimed at enhancing coping strategies and improving family dynamics.

• Adhere to all Federal and State regulations regarding mental health services, ensuring compliance with ethical and legal standards.

• Collaborate actively in interdisciplinary team meetings to promote integrated care and align treatment goals across the healthcare spectrum.

Patient Population / Client Focus\n

This role focuses on supporting senior clients who may be facing chronic illnesses, mental health challenges, or psychosocial issues related to navigating the complexities of aging. You will work directly with individuals in hospice care, providing a compassionate approach to end-of-life matters, helping clients and their families find peace and understanding during difficult transitions.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a related field is mandatory.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in Texas.

• Demonstrated expertise in crisis management, risk assessment, treatment planning, and evaluation for senior populations.

• Strong verbal and written communication skills; ability to convey empathy and support through active listening.

• Ability to manage time effectively and prioritize tasks in a fast-paced environment.

Preferred Experience\n

• At least two years of clinical experience working with elderly clients, preferably within a hospice or outpatient mental health setting.

Compensation and Benefits\n
