Key Responsibilities\n

• Psychosocial Assessment: Conduct thorough, timely assessments that utilize your knowledge of human behavior and psychosocial factors to inform treatment plans and ethical considerations.

• Clinical Intervention: Provide supportive counseling and liaise with families to facilitate difficult decisions, such as hospice care and DNR arrangements. You will also assist in coordinating with community agencies and services for comprehensive care.

• Case Management: Manage a clinical caseload effectively by identifying high-risk situations, balancing patient referrals, and ensuring ongoing support. Collaborate with case managers to optimize discharge planning.

• Documentation: Maintain accurate documentation of patient care, including interdisciplinary plans of care and progress notes, following established policies and ensuring compliance.

• Training and Support: Offer in-service training sessions to hospital staff regarding psychosocial issues related to patient care, contributing to a culture of learning and improvement.

• Quality Improvement: Participate in developing quality improvement initiatives aimed at advancing patient care through the analysis of process and outcome indicators.

• Additional Duties: Support special initiatives, including educational programs for patients and families, and activities related to Social Work Month.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n

The annual base salary for this position ranges from USD 120,000 to 130,000, depending on qualifications and experience.
