Work Environment\n

This role offers the convenience of a remote work environment, allowing the LCSW to connect with clients while minimizing travel barriers. Although the position is predominantly remote, the LCSW will occasionally collaborate with interdisciplinary teams, including nurses and caregivers, through virtual meetings or on-site visits as needed. The typical work schedule spans Monday to Friday, from 8:00 AM to 4:30 PM, with occasional flexibility required for client care during evenings or weekends.

Key Responsibilities\n

The Licensed Clinical Social Worker will engage in a variety of essential responsibilities, including:

• Conducting thorough assessments of seniors' psychosocial needs, incorporating emotional, mental, and social factors into comprehensive care plans.

• Collaborating with clients and their families to design individualized care strategies that address specific goals, strengths, and preferences.

• Providing counseling and emotional support to help seniors navigate significant life transitions, including health challenges, relocation, or loss of independence.

• Advocating for clients’ access to community resources, effectively ensuring they receive the support and services they require.

• Coordinating care with an interdisciplinary team to enhance service delivery and meet clients’ holistic needs.

• Conducting regular home visits or virtual check-ins to monitor clients’ well-being, assessing living conditions, and addressing safety concerns.

• Facilitating communication and conflict resolution among family members involved in the senior client's care.

• Educating clients and their families about community resources, support groups, and programs that foster social engagement and improve quality of life.

• Maintaining accurate and timely documentation of assessments, care plans, and progress notes in accordance with ethical and regulatory standards.

Qualifications\n

Compensation and Benefits\n
