Key Responsibilities\n

• Conduct comprehensive assessments of clients’ psychosocial needs by considering emotional, mental, and social factors to create personalized care plans.

• Engage actively with seniors and their families, providing counseling and emotional support during significant life transitions such as loss, health changes, or relocation.

• Promote clients' rights and facilitate their access to community resources that can aid in their overall well-being and assure they have necessary support services.

• Collaborate with an interdisciplinary team, which includes nurses, physicians, and caregivers, to coordinate multifaceted care approaches that address all aspects of a client’s health.

• Monitor client well-being through regular follow-up sessions, noting changes in mental and emotional health, living conditions, or social interactions.

• Resolve conflicts among family members regarding care decisions, and educate all parties on available resources and support systems.

• Ensure meticulous documentation of client interactions and progress notes to comply with healthcare regulations and to inform future care strategies.

Qualifications\n

Compensation and Benefits\n
