Key Responsibilities\n

In your role as a Licensed Mental Health Counselor, your primary responsibilities will include:

• Conducting thorough diagnoses and treatment of mental, emotional, and behavioral disorders, adapting care plans as necessary to reflect changes in patient status.

• Referring patients to additional resources to enhance their well-being, including community services, outpatient programs, and support groups, ensuring they have access to a wide network of care options.

• Collaborating with a multidisciplinary team to develop effective discharge plans that facilitate continued support for patients post-treatment.

• Adhering to federal, state, and local guidelines as well as organizational policies, fully operating within the scope of the Mental Health Practice Act.

• Prioritizing daily tasks by assessing the specific needs of new, current, and discharging patients to ensure timely and effective care.

• Providing psychosocial assessments and interventions, including crisis support, bereavement counseling, and education for patients and their families.

• Coordinating with various care teams and agencies to guarantee efficient transitions in care and to resolve any existing challenges promptly.

• Advocating on behalf of patients and families to communicate effectively with multidisciplinary teams and ensure all parties are aligned in care strategies.

• Identifying patients at risk for readmission or other barriers to care, working proactively to mitigate potential issues that could impact recovery.

Qualifications\n
