Work Environment

This role is performed on-site in patients' homes, providing a dynamic and engaging work environment that promotes independence and comfort. As a Physical Therapist with Ernest Physical Therapy, you will have the flexibility to travel throughout the community, creating unique therapeutic relationships and directly witnessing the positive outcomes of your intervention. Monday to Friday, you will work a full-time schedule comprising a standard 40-hour workweek, enabling a balanced work-life integration.

Key Responsibilities

As a Physical Therapist in our home care program, you will be responsible for:

• Conducting comprehensive evaluations of patients referred by physicians to develop individualized treatment plans.
• Implementing evidence-based therapeutic interventions that address patients' specific needs and goals, whether they relate to orthopedic, pediatric, neurologic, or sports medicine conditions.
• Demonstrating exceptional customer service and communicating effectively with patients and their caregivers to foster a supportive and encouraging therapeutic environment.
• Collaborating with a multidisciplinary team of healthcare professionals to optimize patient outcomes and ensure comprehensive care.
• Providing ongoing education and training to patients and their families, empowering them with the knowledge to manage their conditions effectively.
• Monitoring and documenting patient progress meticulously, adhering to established care plans and adjusting strategies as necessary.

Patient Population / Client Focus

Your work will primarily focus on patients recovering from surgery, those managing chronic illnesses, or individuals in need of rehabilitative care post-injury. You will play a crucial role in supporting patients at various stages of their recovery journey, applying your expertise to help them regain mobility, improve strength, and restore function. By engaging with patients in their own environments, you will gain deeper insights into their daily challenges and successes, allowing for more tailored care.

Qualifications

To be considered for this position, applicants must possess:

• A valid Doctorate in Physical Therapy (DPT), along with a Bachelor's degree in Physical Therapy.
• A state license to practice physical therapy in Florida.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong communication skills and a willingness to connect with diverse patient populations actively.
• Competence in using technology and electronic health records for documentation and patient management.

Preferred Experience
