Key Responsibilities\n

• Effectively communicate with physicians and multidisciplinary team members to report on patient progress, needs, and concerns, ensuring a holistic approach to treatment.

• Maintain accurate and timely clinical records, including comprehensive progress notes that adhere to department policies and contribute to ongoing patient assessments.

• Collaborate with healthcare team members to develop and refine treatment plans, including discharge planning based on thorough psychosocial assessments and established best practices.

• Provide counseling and crisis intervention tailored to individual patient needs, with a focus on developing strategies for managing emotional and behavioral issues.

• Assist clients with discharge planning, coordinating care with various regional providers to facilitate access to necessary support services.

• Engage with families and support systems to ensure that treatment plans are aligned with patient needs and involve necessary stakeholders in the discharge process.

Patient Population / Client Focus\n

In this role, you will primarily serve adults and adolescents experiencing a range of mental health issues, including anxiety, depression, trauma, and behavioral disorders. Your empathetic approach and expertise will play a pivotal role in guiding clients through their recovery journeys, ensuring that care is tailored to each individual's unique circumstances.

Qualifications\n

• A Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology is required.

• Current licensure as an LMFT, LPC, LCSW, or an equivalent credential is essential.

• Minimum of 3 years of demonstrated experience in an outpatient mental health setting, showcasing your ability to deliver personalized care.

• Proficient in delivering evidence-based therapeutic interventions tailored to client needs and promoting long-term mental wellness.

• Strong organizational skills with the ability to work independently and manage caseloads efficiently in a virtual environment.

Preferred Experience\n

Compensation and Benefits\n
