Description

• Assess patients in either an inpatient or outpatient environment and formulate personalized treatment plans
• Implement physical therapy interventions emphasizing mobility, strength, balance, and function
• Collaborate with nurses, doctors, case managers, and other rehabilitation professionals to optimize patient outcomes
• Educate patients and their families regarding therapeutic goals, discharge processes, and at-home exercises
• Document evaluations, treatments, and progress according to facility protocols and regulatory standards
• Engage in care team meetings and interdisciplinary discussions to ensure seamless care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
