Key Responsibilities\n

• Design and implement personalized, integrated treatment plans, focusing on both mental health and any co-existing medical conditions.

• Provide immediate crisis intervention during mental health emergencies, ensuring that patients receive the support they need promptly.

Patient Population / Client Focus\n

Your work will primarily focus on individuals who experience complex mental health challenges coupled with co-occurring medical conditions. This includes patients managing conditions such as chronic pain, substance use disorders, or other health issues that profoundly influence their mental well-being. Your mission will be to address these intertwined needs and help patients navigate their journey towards improved health.

Qualifications\n

• A Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in New York State.

• Strong foundational knowledge and experience in addressing mental health needs of individuals with interconnected psychiatric and medical conditions.

• Excellent interpersonal and communication skills to establish rapport with patients and collaborate effectively with a multidisciplinary team.

• Ability to maintain professional boundaries while adhering to ethical guidelines and confidentiality standards.

• Proficient with electronic health record systems and relevant computer applications.

Preferred Experience\n

While not mandatory, candidates with extensive experience in integrated care settings or working with patients who have co-occurring disorders will be given preference. Additionally, those familiar with telehealth practices and virtual therapy techniques will find advantages in this role.

Compensation and Benefits\n
