Key Responsibilities\n

• Conduct regular evaluations of clients to assess their mental health needs, tailoring your approach based on individual circumstances.

• Facilitate effective communication among supervisors, direct care staff, and family members, providing progressive updates to keep everyone informed of client treatment plans.

• Develop and share a tentative action plan with supervisors that includes scheduled appointments, home visits, and observation sessions, ensuring accountability and tracking progress.

• Organize and conduct home visits, as necessary, to collect data, perform chart reviews, interview staff, and conduct observations to satisfy specific client needs.

• Ensure precise implementation of the program strategy, guaranteeing all facets of care are executed effectively.
\n
• Actively participate in Interdisciplinary Team meetings to facilitate comprehensive client assessments and multidisciplinary support.
\n
• Prepare monthly and quarterly progress reports that summarize patient progress and outcomes, utilizing these reports to adjust treatment strategies as needed.

• Provide training sessions for team members to enhance the proficiency and effectiveness of program execution and documentation.
\n
• Conduct follow-up interviews, home visits, and observations to continually address the evolving needs of your clients.

• Maintain client records systematically to ensure information is readily available to those in need.

• Serve as a vital link between the broader community and mental health services, aiding individuals in achieving personal development goals.
