Key Responsibilities\n

The Licensed Professional Counselor will have a variety of essential duties aimed at supporting our patients' mental health needs:

• Conduct comprehensive assessments to evaluate the mental health needs of individuals, ensuring tailored therapeutic interventions.

• Maintain open lines of communication with supervisors, direct care staff, family members, and other professionals regarding patient progress, ensuring a collaborative care approach.

• Organize a tentative schedule of appointments and home visits to facilitate thorough monitoring and support throughout the patient's journey.
\n
• Manage home visits to perform in-home observations, review patient charts, and conduct staff interviews focused on collecting relevant data for ongoing improvement.

• Implement and monitor the effectiveness of individualized program plans, adjusting them as necessary to ensure they meet each client's evolving needs.

• Participate actively in interdisciplinary team meetings, contributing insights and updates on patient progress and therapeutic strategies.

• Document monthly and quarterly progress notes to track patient outcomes and inform ongoing treatment plans.
\n
• Provide training for staff and team members on program implementation and necessary documentation, ensuring compliance with best practices.
\n
• Conduct follow-up assessments and home visits to reinforce support mechanisms and ensure patient needs are continuously met.

• Keep comprehensive records to provide clear, accessible information to stakeholders, enhancing transparency and timeliness of care.

Patient Population / Client Focus\n

The LPC will work with a diverse client population, including adults and youth facing an array of mental health challenges. This includes, but is not limited to, anxiety, depression, trauma, substance misuse, and family dynamics. The counselor will provide compassionate, client-centered care that respects each individual's unique experiences and background.

Qualifications\n

To succeed in this role, candidates must possess the following qualifications:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Arizona.

• A minimum of two years of relevant counseling experience, preferably in a clinical setting.

• Specialization in family therapy, couples therapy, trauma-focused therapy, or substance use treatment is highly desirable.

• A strong understanding of applicable legal and ethical standards in the counseling profession.

• Exceptional interpersonal and communication skills, with the ability to build rapport with diverse clientele.

• Demonstrated ability to collaborate effectively with a multidisciplinary healthcare team.

• Competency in using electronic medical records (EMR) software and related technologies to manage patient information.

Preferred Experience\n

Preferred candidates will have experience working within frameworks that foster trauma-informed care and demonstrate a strong ability to integrate evidence-based practices in their therapeutic approaches. Familiarity with community resources and the mental health landscape in Arizona will also be beneficial.

Compensation and Benefits\n
