Key Responsibilities\n

• Conduct comprehensive evaluations and ongoing assessments to ensure that each patient's unique needs are met throughout their care journey.

• Facilitate effective communication with supervisors, direct care staff, families, and guardians to ensure a cohesive understanding of each individual's developmental progress.

• Develop and share detailed treatment plans and progress reports with supervisors, outlining appointments, home visits, and relevant observations to help track client progress.

• Organize and execute home visits to perform in-home assessments, conduct chart reviews, and interact with patients and their families, ensuring their needs are fully understood.

• Ensure the effective implementation of therapy programs while participating in Interdisciplinary Team Meetings for collaborative care planning.

• Maintain accurate monthly and quarterly progress notes reflecting treatment outcomes and changes in patient status.
\n
• Conduct follow-up services through interviews and visits to assess the sustained needs of clients and adjust care plans as necessary.

• Regularly update and maintain client records in accordance with current regulations and best practices to guarantee accessibility for all involved parties.

• Act as a liaison between the clinic and the community, promoting mental health awareness and fostering relationships among stakeholders.

Patient Population / Client Focus\n

The individuals you will serve come from a variety of backgrounds, including both adults and youth. Your expertise in specialized areas, such as family therapy or trauma-informed care, will be invaluable as you help clients navigate their mental health challenges. More specifically, our patient population may include individuals coping with substance misuse disorders, anxiety, depression, and interpersonal issues.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

• A minimum of two years of clinical counseling experience, with a strong preference for those versed in outpatient therapy contexts.
