Key Responsibilities\n

• Collaborate with primary care providers to identify and address mental health issues, acting as an integral part of the patient care team.

• Conduct comprehensive patient assessments that include diagnostic and functional evaluations, offering tailored recommendations to the primary care team.

• Deliver impactful and focused therapeutic interventions to patients requiring mental health support, ensuring that these are aligned with evidence-based practices.

• Guide primary care providers on the appropriate classification of patients for either primary care or specialized mental health treatment, optimizing individual care pathways.

• Initiate follow-up procedures to monitor patient progress, adjusting treatment plans as necessary based on ongoing assessments.

• Develop relapse prevention plans, supporting clients in maintaining stable mental health and functioning.
\n
• Identify at-risk patients and intervene with preventive strategies to mitigate the exacerbation of their mental health conditions.

• Document all patient interactions meticulously in treatment charts, adhering to legal and ethical healthcare standards.

• Maintain effective communication with primary care staff to ensure cohesive treatment planning and execution.

• Educate patients on self-management strategies to empower their active participation in recovery, effectively improving their overall health outcomes.

• Provide consultancy support to clinic management on behavioral health services and contribute to program evaluation strategies.

• Support patients in comprehending and adhering to their medical treatment plans initiated by other healthcare providers.

Qualifications\n

Compensation and Benefits\n

The annual salary for this position ranges from USD 87,000 to 105,000, depending on experience and qualifications.
