Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments for our diverse patient population, identifying their mental health, substance abuse, and medical needs within the primary care framework.

• Provide individualized and group counseling services to both adults and children, facilitating sessions focused on mental health issues such as anxiety, depression, and grief.
\n
• Perform intake assessments, develop treatment plans, and provide crisis intervention as necessary, ensuring that patients receive the appropriate higher-level care when needed.
\n
• Document all mental health evaluations, treatment progress notes, and suicide risk assessments accurately in our electronic health record (EHR) system, allowing for seamless communication among care providers.

• Foster collaborative relationships with other healthcare providers and facilitate referrals to community resources, including medication assistance and rehabilitation programs.
\n
• Work actively with Quality Improvement teams to refine and implement data collection processes, ensuring high-quality, evidence-informed practice within the organization.

Patient Population / Client Focus\n

As an LCSW in our Hospice Center, you will primarily work with children and families coping with serious illnesses and end-of-life issues. Your expertise will be essential in providing culturally competent care tailored to the unique emotional and psychological needs of our clients. Your ability to build trust and rapport will enhance their coping mechanisms and overall quality of life during challenging times.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Active state license as a Licensed Clinical Social Worker (LCSW).

• A minimum of three years of post-licensure experience in a clinical setting.

Compensation and Benefits\n
