Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments tailored to the unique needs of patients in hospice care, focusing on mental health, substance abuse, and medical concerns.

• Provide therapeutic services that include individual and group counseling, intake evaluations, and crisis intervention, allowing clients to process their experiences in a safe environment.

• Develop detailed treatment plans in collaboration with interdisciplinary teams that address the multifaceted needs of patients and their families.

• Document all services electronically, including progress notes, treatment plans, and compliance with legal and ethical standards.

• Facilitate referrals to community resources, ensuring patients have access to essential support services, including medication assistance programs and local support groups.

• Engage with Quality Improvement teams to refine data collection processes and enhance service delivery effectiveness.

Qualifications\n

• Active Licensure as a Clinical Social Worker (LCSW) in Oregon is required.

Compensation and Benefits\n
