Work Environment\n

This role will be performed in a remote work setting, allowing for flexibility while maintaining a crucial link to our patients. While the position operates primarily from home, the LCSW will conduct regular check-ins and assessments, ensuring a continuous connection with clients. Full-time hours are Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends as needed based on client requirements. The nature of hospice care requires an openness to on-call situations for urgent intervention.

• Advocate for clients by ensuring they have access to community resources and necessary support services.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Possession of a current and valid state licensure as a Clinical Social Worker (LCSW).
