Work Environment

The FNP will work onsite at our assisted living facility, where you will have the opportunity to build meaningful relationships with patients and their families. Your standard work hours will typically be from 9:00 AM to 6:00 PM, with occasional evening clinics extending until 8:00 PM. Participation in community outreach efforts and health fairs on weekends will enable you to engage with the wider community and promote health awareness. Your average workweek will total approximately 40 hours, facilitating a balance between professional and personal commitments.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients, utilizing evidence-based practices to inform diagnoses.
• Manage both acute and chronic medical conditions, emphasizing preventive care, health promotion, and patient education tailored to individual needs.
• Collaborate with local community organizations to coordinate health education programs, ensuring that resources are accessible to all community members.
• Provide culturally sensitive care by understanding and respecting the diverse backgrounds of patients and their families, advocating consistently for patients' rights.
• Facilitate referrals and follow-up care to ensure patients receive the comprehensive support necessary for optimal health outcomes.
• Maintain accurate and detailed medical records in compliance with industry regulations, ensuring timely documentation of patient interactions and treatments.
• Engage in community outreach initiatives and health fairs to extend your impact beyond the facility, providing critical screenings and health education to the public.

Patient Population / Client Focus

The primary client focus for this role includes elderly residents of our assisted living facility, many of whom contend with chronic health issues and complex medical needs. Additionally, your outreach efforts will target local underserved populations, providing essential health services to improve overall public health and wellbeing in Corpus Christi.

Qualifications

• Current certification as a Nurse Practitioner, with an emphasis on Family Practice.
• An active nurse practitioner license issued by the state of Texas.
• A Bachelor’s degree in nursing, and a Master's degree in nursing from an accredited program, showcasing your advanced understanding of healthcare practices.
• A minimum of 4 years of clinical experience, ideally in community health or public health settings, where you have honed your patient care skills.
• Exceptional interpersonal skills and communication abilities, enabling effective interaction with diverse patient populations and their families.

Preferred Experience

• Background in community health initiatives and direct engagement with vulnerable populations.
• Experience with electronic health record (EHR) systems to facilitate seamless patient data management and communication.

Compensation and Benefits

This position offers a competitive yearly salary ranging from $120,000 to $220,000, based on your qualifications and experience.
