Key Responsibilities

• Conduct comprehensive assessments to evaluate the patient's physical abilities and limitations, ensuring that each treatment plan reflects their individual recovery goals.
• Design and implement personalized rehabilitation plans that include hands-on therapy techniques aimed at restoring function, mobility, and strength.
• Provide one-on-one patient therapy sessions in the home setting, creating a comfortable environment conducive to healing and recovery.
• Utilize various evidence-based treatment modalities, including manual therapy and therapeutic exercises, to facilitate effective rehabilitation outcomes.
• Communicate regularly with patients and their families about progress, treatment goals, and recommendations for continued care.
• Maintain accurate and timely documentation of patient interactions, treatment plans, and progress notes using EMR systems (experience with WebPT is advantageous).
• Advocate for patients' needs by exploring non-surgical treatment options and providing education on managing their conditions effectively.
• Foster a supportive and motivating environment, encouraging patients to engage in wellness activities that enhance their overall health.

Patient Population / Client Focus

In this role, you will work primarily with a diverse patient population, including individuals recovering from orthopedic surgeries, chronic pain, and general mobility challenges. Your approach will be tailored to each patient's age, cultural background, and specific health conditions, ensuring that care is inclusive and respectful of their unique circumstances.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Active and unrestricted Physical Therapist license in California.
• A minimum of two years of relevant clinical experience in a physical therapy setting, preferably with home care experience.
• Proficient in manual therapy techniques and familiar with rehabilitation protocols for diverse patient populations.
• Strong interpersonal skills to foster relationships with patients and their families while promoting a positive and empathetic atmosphere.

Preferred Experience

• Experience with EMR systems such as WebPT for documentation and patient management.
• Familiarity with developing community-based wellness programs.
• Continuing education certifications in specialized therapy techniques or modalities.

Compensation and Benefits

The compensation for this position ranges between $110,000 and $125,000 annually, commensurate with experience and qualifications. Our comprehensive benefits package includes:
