Key Responsibilities\n

• Conduct comprehensive assessments that identify clients’ unique social, emotional, and environmental challenges, ensuring a tailored approach to their needs.

• Collaborate closely with community organizations and agencies to coordinate services, taking on the role of advocate and resource connector for clients.

• Participate in community outreach initiatives aimed at raising awareness of available services and building rapport with local partners.

• Take part in regular case conferences, team meetings, and continuous staff training to enhance your professional skills and contribute to team synergy.

Qualifications\n

Compensation and Benefits\n
