Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families to identify emotional, social, and environmental strengths and challenges related to their medical condition and life circumstances.

• Formulate, develop, and implement personalized treatment plans that utilize appropriate social work interventions, reflecting each patient's unique needs.

• Act as a consultant and resource within the organization by providing education and guidance around social work best practices to team members and other healthcare professionals.

• Collaborate with the healthcare team to ensure comprehensive planning and implementation of care, involving both patients and families in decision-making processes.

• Facilitate seamless discharge planning by engaging with consulting physicians, third-party payers, and various healthcare providers to establish an effective and timely transition for patients post-hospitalization.

• Offer education and counseling to patients and families, emphasizing adaptation strategies for dealing with illness and hospital experiences.

• Utilize crisis intervention techniques as required, which may include individual, group, and family therapies tailored to short-term or long-term needs.

• Participate in on-call rotations to address after-hour issues, ensuring continuous support and crisis management when needed.

• Independently facilitate involuntary examinations under the Baker Act when clinically necessary, ensuring proper transfer to the relevant receiving facilities.

• Present findings and techniques at educational conferences, acting as a subject matter expert in psychosocial oncology practice.

• Establish community connections through education, consultation, research, and volunteering, enhancing resources available to cancer survivors and promoting their overall well-being.

Compensation and Benefits\n

This role offers a competitive salary range of $70,000 to $100,000 annually, commensurate with experience and qualifications.
