Key Responsibilities\n

• Conduct thorough assessments to identify client needs and facilitate effective referrals to relevant community-based services.

• Provide immediate support within a crisis-driven environment, leveraging your knowledge of substance abuse, domestic violence, trauma, and various mental health conditions.

• Educate multidisciplinary team members on psychosocial stressors impacting our clients to drive collaborative treatment planning and improve health outcomes.

• Create and maintain detailed clinical records, including patient histories, assessments, treatment plans, diagnoses, and progress notes utilizing electronic health record systems.

Patient Population / Client Focus\n

In your role as a Mental Health Counselor, you will work primarily with a diverse clientele, including children, adolescents, adults, and families experiencing a range of mental health challenges. Qualifications\n
