Work Environment

As a full-time Physical Therapist at Vitality Care Clinic, you will be primarily working onsite, providing direct care to patients in their homes. Key Responsibilities

Your day-to-day responsibilities will include:

• Conducting comprehensive Physical Therapy evaluations to assess patients' needs and challenges.
• Developing and implementing personalized treatment plans tailored to each client's goals, incorporating evidence-based practices.
• Regularly monitoring patients' progress through planned reassessments, modifying treatment strategies as necessary to ensure optimal outcomes.
• Maintaining accurate and timely documentation of patient encounters, including SOAP notes and progress summaries, utilizing our electronic record system.
• Providing education and support to patients and their families about their recovery journey, addressing concerns and challenges they may encounter.
• Meeting established utilization goals and other performance metrics to support the clinic's overall objectives.
• Collaborating with colleagues across various disciplines, particularly for clients receiving multiple therapy services, to enhance the integrated care approach.
• Acting as a representative of the clinic, embodying our core values, and promoting a positive workplace culture.

Patient Population / Client Focus

You will be working with a diverse patient population, including children and adults from various backgrounds with a wide range of diagnoses. This role requires a deep understanding of developmental and rehabilitation challenges, as well as a commitment to providing compassionate care that respects each individual's unique circumstances and goals.

Qualifications

The ideal candidate should meet the following qualifications:

• A Bachelor's degree in Physical Therapy from an accredited program is required; a Master's or Doctorate degree is preferred.
• A current and unrestricted physical therapy license issued by the Board of Physical Therapy in the state of California.
• Willingness to provide evaluations and treatments both in person and via telehealth, depending on clinical appropriateness.
• A strong commitment to working with a diverse patient demographic, understanding cultural competencies and sensitivities.
• The ability to be flexible and adaptable in a rapidly changing environment.
• A focus on collaboration, teamwork, and exceptional customer service.

Preferred Experience

Experience in home care physical therapy settings is highly desirable. Familiarity with telehealth protocols and patient engagement strategies is a plus. Previous work or internship experience with diverse populations will be advantageous in understanding the unique needs of our patients.

Compensation and Benefits
