Work Environment

This position is primarily onsite, allowing the Physical Therapist to provide personalized care in the comfort of patients' homes. Operating Monday through Friday, our dedicated facility is open from 8 AM to 3 PM, with flexibility for evening appointments as necessary.

Key Responsibilities

• Conduct comprehensive Physical Therapy evaluations for patients of all ages, assessing their physical capabilities, limitations, and rehabilitation needs.
• Develop individualized treatment plans based on patient evaluations, tailoring interventions to promote functional independence and enhance quality of life.
• Monitor patient progress by regularly reassessing clinical signs and symptoms, adjusting treatment plans as necessary to ensure optimal outcomes.
• Document all patient interactions accurately using the electronic health record system, completing SOAP notes, progress summaries, and care plans in a timely manner.
• Educate patients and their families about therapy processes, expected challenges, and strategies to facilitate recovery outside of scheduled sessions.
• Meet minimum utilization goals set by the organization while contributing to overall business objectives.
• Engage in interdisciplinary collaboration with healthcare colleagues within the organization for patients receiving multiple therapy services.

Patient Population / Client Focus

As a Physical Therapist at Vitality Care Clinic, you will serve a diverse patient population, including clients ranging from pediatrics to geriatrics. This includes engaging with individuals presenting various health challenges such as musculoskeletal issues, neurological conditions, post-operative recovery, and rehabilitation following injuries. Qualifications

• A Master's Degree or Doctorate in Physical Therapy from an accredited institution.
