Work Environment\n

This role is primarily remote, allowing for flexibility to manage your schedule while addressing the needs of clients effectively. While most assessments and counseling will take place virtually, you will have opportunities for in-person visits to assess clients directly, evaluate living conditions, and provide essential support. Our team operates from Monday to Friday, with the possibility of evening and weekend hours as required by client needs. You will need to be adaptable in your scheduling to ensure that clients receive timely assistance.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial statuses, considering their emotional, mental, and social well-being.

• Develop personalized care plans collaboratively with clients and their families, ensuring alignment with their specific goals and personal preferences.

• Provide individual counseling and emotional support, guiding seniors through life transitions, such as health changes, loss of loved ones, or relocation to assisted living.

• Advocate for clients to secure access to necessary community resources, including healthcare services, transportation, and social interventions.
\n
• Engage in interdisciplinary teamwork by collaborating with healthcare professionals, including nurses and caregivers, to ensure coordinated comprehensive care.

• Conduct regular follow-up check-ins either virtually or in-person to monitor changes in clients’ well-being and address any pressing issues.
\n
• Facilitate family meetings to mediate any disputes or concerns regarding client care and encourage family involvement in the caregiving process.

• Educate clients and their families on various community support networks, including support groups and relevant social programs, to instigate social interactions and reduce isolation.

• Maintain accurate, confidential documentation of client assessments, care plans, and progress notes in compliance with organizational policies and HIPAA regulations.

• Stay informed on best practices in social work, psychosocial interventions, and current laws affecting the community, ensuring ethical treatment standards are upheld.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

• Current state licensure as a Licensed Master Social Worker (LMSW).

Compensation and Benefits\n
