Key Responsibilities\n

• Conduct thorough psychosocial assessments by engaging with patients through interviews and utilizing observational techniques to gather insightful information regarding their emotional, social, and environmental challenges impacting recovery.

• Deliver counseling and psychological support informed by a deep understanding of adjustment issues, grief, and coping strategies. Actively listen to patients and families to address their emotional health needs.

• Work closely with the multidisciplinary rehabilitation team to create and implement individualized treatment plans that reflect each patient’s unique psychosocial needs and objectives.

• Assist patients in accessing vital community resources, including but not limited to support groups, vocational rehabilitation opportunities, and housing assistance, enhancing their reintegration into the community.

• Provide education and support to patients and their families about community resources and post-rehabilitation options to ensure continuity of care and emotional support.

• Participate actively in discharge planning processes, helping to ensure seamless transitions for patients moving to home or other healthcare settings.

Qualifications\n
