Full-Time Nurse Practitioner for Concierge Medicine Practice

Position Overview

Pulse Point Internal Medicine in Palmyra, New York, is seeking a dedicated and compassionate full-time Nurse Practitioner to join our innovative team. In this role, you will play a vital part in delivering high-quality, patient-centered primary care to adult patients, particularly those on Medicare. You will engage closely with patients, collaborating independently and alongside physicians to develop individualized care plans that promote well-being and prevent hospitalization.

Work Environment

This role is onsite at our Palmyra location, where you will work intimately within a collaborative care team, which includes physicians, nurse practitioners, physician assistants, medical assistants, and support staff such as dedicated medical scribes and care managers. Key Responsibilities

• Provide exceptional primary care services, conducting both routine and acute care office visits.
• Administer annual wellness visits and comprehensive health risk assessments, ensuring a holistic and well-documented approach to patient health.
• Facilitate effective care coordination, liaising with specialists, testing facilities, and community resources to streamline patient care.
• Engage in population health leadership, coordinating with the care team to promote evidence-based screenings and preventive care measures.
• Assist in phone triage and outreach to ensure timely responses to patient inquiries and needs.
• Educate patients about their health conditions, care plans, and treatment options, empowering them to take ownership of their health.
• Participate in promotional activities to enhance community awareness of services.
• Conduct home visits as necessary to provide personalized care in the patient’s environment.
• Report directly to the Medical Director, maintaining close collaboration with operational leadership.

Patient Population / Client Focus

Our primary focus is on providing individualized care for older adults on Medicare. Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• Possess an active, non-probationary state Nurse Practitioner license and DEA license.
• A minimum of two years of experience in primary care settings, preferably in internal medicine, geriatrics, or family medicine.
• Knowledge of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs) is essential.
• A genuine passion for teamwork and a desire to engage in cross-functional collaboration.
• Fluency in both English and an additional language is a plus, enhancing communication with diverse patient backgrounds.

Preferred Experience

While not mandatory, experience in implementing population health initiatives and familiarity with electronic health record (EHR) systems will be beneficial in this role, as they are critical in ensuring effective patient management and care coordination.

Compensation and Benefits

• Competitive salary ranging from USD 180,000 to 200,000 annually, contingent upon experience and demonstrated expertise.
