Work Environment

This role is based onsite in our state-of-the-art medical facility and includes making house calls, ensuring flexible and convenient care delivery for our patients. Our work culture is collaborative and supportive, emphasizing teamwork and mutual respect among staff and patients alike. You can expect a structured schedule primarily from 9:00 a.m. to 6:00 p.m., with occasional evening hours for extended clinics. Participation in various community outreach events may also require weekend availability.

Key Responsibilities

• Perform thorough health assessments, including physical examinations and diagnostic screenings, to evaluate the well-being of patients.
• Diagnose and manage a range of acute medical conditions alongside chronic illnesses, from diabetes management to hypertension.
• Design and implement customized health promotion and disease prevention initiatives in collaboration with community organizations.
• Advocate for patient rights and ensure equitable access to healthcare services for all demographics, fostering an inclusive environment.
• Coordinate with other healthcare professionals for referrals and follow-up care, ensuring continuity and quality of treatment.
• Maintain meticulous and compliant medical records to align with healthcare regulations and standards.

Qualifications

• Current certification as a Family Nurse Practitioner and an active state nursing license.
