Key Responsibilities\n

As a Licensed Professional Counselor at Pathways Wellness Center, you will be responsible for a range of critical tasks that ensure high-quality support for our patients. Your day-to-day responsibilities will include:

• Collaborating with a multidisciplinary healthcare team to assess patient progress, discuss treatment needs, and address any arising concerns.

• Maintaining meticulous and timely clinical documentation, including progress notes and care plans, in compliance with our organizational policies.

• Developing individualized treatment plans in conjunction with other healthcare providers based on thorough psychosocial assessments and established therapeutic interventions.

• Providing crisis intervention and counseling to patients experiencing emotional or behavioral challenges, utilizing evidence-based practices during sessions.
\n
• Assisting clients with discharge planning and facilitating access to follow-up services by working closely with regional providers.
\n
• Engaging with family members and support networks to ensure comprehensive understanding and coordination around the client's treatment and discharge plans.

Qualifications\n

The ideal candidate will possess the following qualifications:
