Key Responsibilities\n

• Conduct therapeutic sessions, both in office and community settings, addressing the needs of clients experiencing major mental illness or severe emotional disorders.

• Develop integrated care plans that facilitate access to essential services, and engage in continuous collaboration and information sharing with clients, their families, and other healthcare providers.
\n
• Participate effectively as a team member, contributing towards team goals and enhancing the service delivery within the organization.
\n
• Perform brief, evidence-based screenings and assessments, determining the necessity for more extensive evaluations when appropriate.
\n
• Provide focused prevention, treatment, and recovery services, including individual, group, and family therapy, while ensuring these services align with the identified needs of clients.
\n
• Assist with crisis intervention and escalation procedures, using evidence-based strategies to maintain client safety and well-being.
\n
• Carry a manageable caseload, ensuring timely completion of client documentation, including assessment reports and progress notes.
\n
• Engage with clients and their families to monitor meeting attendance, service utilization, and ensure discharge criteria are met.
\n
• Participate in agency committees aimed at improving service delivery and quality of care for the population served.

This role specifically caters to vulnerable populations facing significant mental health challenges, including those within the hospice context. You will work with clients and their families dealing with grief, terminal illness, and psychosocial stressors. Qualifications\n

• A master's degree in social work from an accredited institution is a must.

• Active LCSW licensure in the state of Oregon.

• Demonstrated understanding of mental health assessments, treatment planning, and crisis management.

• Excellent verbal and written communication skills, along with demonstrated organizational abilities.

• Ability to represent the organization effectively, maintaining professionalism in all interactions with clients, families, and colleagues.

• Ability to work collaboratively with diverse populations, respecting their unique traditions and beliefs.

Preferred Experience\n

Ideally, candidates will have at least two years of clinical experience, particularly in mental health or community health settings. Familiarity with hospice care and end-of-life issues is highly advantageous, as you will be engaging with clients during some of the most challenging moments in their lives.

Compensation and Benefits\n

The salary range for this position is between USD 70,000 and 85,000 annually, commensurate with experience and qualifications.
