Work Environment\n

This role can be performed remotely, allowing for flexibility in managing work-life balance while still providing excellent service to our clients. Working hours are typically Monday through Friday, from 8:00 AM to 4:30 PM. However, flexibility is crucial as there may be occasional evening or weekend hours required to meet the needs of clients. Furthermore, on-call availability may be necessary to address urgent situations, ensuring continuous support for our clients and their families.

Key Responsibilities\n

• Conduct thorough assessments of seniors' psychosocial needs, evaluating emotional, mental, and social factors impacting their well-being.

• Collaborate with seniors and their families to create personalized care plans that align with their unique goals and preferences.
\n
• Provide counseling and emotional support, assisting seniors and their families in navigating life transitions, such as health challenges or changes in living situations.
\n
• Advocate for clients' rights and facilitate access to community resources, ensuring they receive comprehensive support services.
\n
• Engage in interdisciplinary collaboration with healthcare providers, nurses, and caregivers to coordinate and enhance client care.
\n
• Conduct regular monitoring visits to assess living conditions, observe client interactions, and address any safety or care-related concerns.

• Facilitate communication and mediate any conflicts among family members regarding caregiving responsibilities and care strategies.
\n
• Educate clients and their families about available community resources and provide information on support groups and engagement programs to promote social interaction.

• Maintain meticulous documentation of client assessments, care plans, and progress notes, adhering to ethical guidelines and compliance with regulatory policies.

• Continuously update knowledge of social work practices and community resources to enhance service delivery and outcomes.

Qualifications\n

• Master’s degree in Social Work (MSW) from an accredited university.

• Familiarity with Electronic Health Record (EHR) systems and other relevant software tools.

Compensation and Benefits\n
