Work Environment

The Senior Home Care Physical Therapist role is conducted in a dynamic environment, interacting directly with patients in their homes. This onsite setting allows for personalized assessments and treatment implementations tailored to each patient's unique circumstances. Working Monday to Friday from 9 AM to 5 PM, you will be an essential part of a multidisciplinary team focused on achieving optimal health outcomes through collaborative practices.

Key Responsibilities

• Conduct comprehensive evaluations of patients to assess their physical conditions, mobility limitations, and the impact of their health status on daily activities.
• Develop individualized physical therapy plans of care that align with patients' goals, medical histories, and functional capabilities.
• Implement evidence-based therapy interventions to improve patients' movement abilities, reduce pain, and restore function, adapting approaches based on ongoing evaluations.
• Communicate effectively with healthcare teams, including supervisors and nursing staff, to report patient progress, schedule appointments, and manage administrative tasks related to patient care documentation.
• Educate patients and their families about therapeutic exercises, assistive devices, and home care strategies to enhance recovery and ongoing health.
• Ensure compliance with all regulatory, licensing, and accreditation requirements related to documentation and patient care.
• Maintain a safe, clean, and organized work environment, including regular checks and maintenance of therapeutic equipment.
• Adhere to the company's Code of Business Conduct and maintain high professional standards in all interactions.

Patient Population / Client Focus

You will primarily work with a diverse patient population suffering from orthopedic issues, musculoskeletal conditions, sports injuries, and rehabilitation needs stemming from surgeries, chronic illnesses, or injuries. Your role will involve assessing patients' unique circumstances, thus allowing for targeted interventions that promote both recovery and long-term health maintenance.

Qualifications

• Valid Physical Therapy licensure in good standing in the State of California.
• National Provider Identifier (NPI) number required.
• Graduation from an accredited Physical Therapy program.
• Minimum of two years of experience in physical therapy, particularly in orthopedic and sports rehabilitation.
• Demonstrated knowledge of contemporary evidence-based practices, therapeutic exercises, and the ability to design programs that cater to different ability levels and comorbidities.
• Exceptional communication skills to foster effective patient education and team collaboration.
• Capability to work autonomously and as part of a multidisciplinary team focused on integrated patient care.

Preferred Experience

• Prior experience in developing training materials and best practice documentation.
• Exposure to electronic medical record (EMR) systems and familiarity with documentation processes.
• Experience working in home health or community-based settings is advantageous.

Compensation and Benefits

We offer a competitive compensation package, ranging from $100,000 to $125,000 annually, commensurate with experience. In addition to a salary, our benefits package includes:
