Work Environment

As a Physical Therapist within our Home Care division, you will work onsite, providing rehabilitation services directly in patients' homes. Key Responsibilities

• You will conduct thorough patient assessments to evaluate therapeutic, rehabilitative, and functional statuses.

• Provide hands-on physical therapy treatments using evidence-based techniques consistent with the physician's plan of care.
• Educate patients, families, and caregivers on therapies, exercises, and the use of rehabilitation equipment to ensure continuity of care outside of scheduled sessions.
• Coordinate care with physicians and other healthcare team members to promote a holistic approach to patient rehabilitation.
• Manage your caseload effectively by prioritizing patient needs and determining daily staffing requirements for optimal care delivery.

Patient Population / Client Focus

In this role, you will primarily work with a diverse patient population that includes both adults and children recovering from surgery, injury, or chronic conditions. Your expertise will play a crucial role in helping patients regain functional independence and improve their quality of life through tailored therapeutic interventions.

Qualifications

• Licensed Physical Therapist in the state of California.
• A Doctor of Physical Therapy (DPT) degree.
• Basic Cardiac Life Support (BCLS) certification, or the ability to obtain it within 30 days of your start date.
• No previous experience is required, making this an excellent opportunity for recent graduates and those early in their careers.

Preferred Experience
