This position is tailored for a remote work setting within a school facility context. Regular business hours will primarily dictate your schedule, with occasional evening or weekend shifts required based on the needs of your patients and caseloads.

Key Responsibilities\n

• Conduct thorough psychosocial assessments that encompass interviews, direct observations, and comprehensive reviews of patient medical records. This process helps identify emotional, social, and environmental influences that may impact their recovery.

• Educate patients and their families about available community resources and essential post-rehabilitation services, ensuring a seamless transition to life outside clinical settings.

• Actively participate in discharge planning processes, facilitating effective transitions to home-based or alternative care settings.

You will be working with individuals across various age groups who face challenges stemming from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Understanding their unique emotional and social needs is vital to ensuring successful rehabilitation outcomes.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Must possess an active state license as a Licensed Clinical Social Worker (LCSW).
