Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the emotional, social, and financial challenges faced by clients.

• Develop and implement individualized treatment plans that include counseling interventions, resources, and referral actions.

• Provide individual and group therapy sessions focusing on issues such as adjustment disorders, grief, anxiety, and coping strategies.

• Collaborate closely with interdisciplinary teams to optimize patient outcomes, contributing your specialized knowledge on social determinants of health.

• Facilitate crisis intervention services, offering immediate support and de-escalation strategies for clients in distress.

• Coordinate with external community organizations to ensure comprehensive post-discharge services for clients.
