Work Environment\n

This position is remote but will work closely with school staff and other healthcare professionals to deliver care that meets the needs of students. Key Responsibilities\n

• Build rapport and communicate effectively with students, families, school staff, and healthcare team members, utilizing active listening skills and adaptive communication styles.

• Participate as an integral member of a professional team that includes behavioral health specialists and primary care providers, ensuring a unified approach to healthcare.

• Conduct evidence-based screenings for various mental health conditions, including anxiety, depression, and substance abuse, and arrange for comprehensive assessments as necessary.

• Create and manage a caseload of students, providing follow-up interventions and support tailored to individual needs.
\n
• Utilize electronic medical records to streamline healthcare delivery, ensuring documentation is completed within 48 hours of patient encounters to enhance integrated care.

• Develop and implement care plans that connect students with necessary services and facilitate communication among all stakeholders, including families and healthcare providers.
\n
• Maintain high standards of patient confidentiality and privacy in all communications and documentation.
\n
• Maintain active participation in supervision, case conferences, and quality improvement initiatives to continuously elevate service quality.
\n
• Facilitate referrals to psychiatric providers for students prescribed medication, ensuring comprehensive care management.
\n
• Provide timely responses to inquiries from students and families regarding appointments and concerns.
\n
• Identify and link students to community resources such as educational, vocational, and support services that align with their goals.

• Perform additional duties as required by the needs of the organization.

Patient Population / Client Focus\n

This role primarily focuses on a diverse population of students, including those from underserved and homeless backgrounds. Qualifications\n

• A Master's degree in a related Behavioral Health field is required.

• A valid LCSW license in the state of Pennsylvania is mandatory.
