Key Responsibilities

• Conduct thorough evaluations, gathering pertinent patient histories and implementing specialized assessments to develop tailored treatment plans.
• Communicate evaluation findings and proposed treatment plans to both patients and families, involving them actively in the goal-setting process.
• Proficiently implement treatment protocols that are responsive to the evaluations and adapt to the evolving healthcare landscape.
• Collaborate with other healthcare professionals as needed, documenting changes in patient conditions and making necessary adjustments to the care plan.
• Manage discharge planning and follow-up care, ensuring that documentation is completed promptly and thoroughly to reflect the patient's progress.
• Demonstrate effective time management skills, ensuring adherence to productivity standards and maintaining an accurate appointment schedule.
• Engage in departmental initiatives to meet organizational goals, including compliance with industry standards and regulations.

Patient Population / Client Focus

Your work will primarily focus on patients requiring rehabilitative care in their homes, including post-operative recovery, rehabilitation for chronic conditions, and recovery from injuries. You will develop personalized care plans that align with the unique needs and lifestyles of each patient, emphasizing recovery and independence.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current license to practice as a Physical Therapist in Florida, in accordance with state regulations.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Though prior work experience is not required, a passion for patient care is essential.
