Work Environment

This role is primarily located in patients' homes throughout the beautiful community of West Palm Beach. As an onsite provider, you will travel to various residences, where you will effectively assess and treat patients in their natural living environments. This setting offers the advantage of allowing you to observe how individual circumstances may impact rehabilitation outcomes.

Key Responsibilities

• Conduct thorough evaluations of patients by reviewing health histories, performing specific assessments, and establishing a baseline for therapy.
• Communicate evaluation findings and tailored treatment plans clearly to patients and their families, ensuring their involvement in goal-setting for rehabilitation.
• Implement evidence-based treatment protocols based on patient assessments and adapt strategies according to their evolving health conditions and responses to therapy.
• Collaborate and consult with other healthcare professionals when necessary, promptly recognizing and documenting changes in patients' conditions that may require an adjustment in care plans.
• Oversee appropriate discharge planning and aftercare, meticulously documenting each step of the patient care process, from initial evaluations to final follow-ups, ensuring compliance with healthcare regulations.
• Manage time effectively to meet productivity targets, maintain accurate scheduling, and participate in team meetings punctually, enriching collaborative practice.
• Contribute actively to departmental goals and compliance with all regulatory requirements, fostering a team-oriented approach within the clinic.
• Pursue continuous professional education and development, mentoring and guiding student interns as a clinical instructor.
• Enhance patient experience through adaptable service, demonstrating flexibility in meeting diverse home care needs.
• Engage in initiatives aimed at removing barriers to care, interpreting site-specific data effectively to drive improvement and access in service delivery.

Patient Population / Client Focus

You will be working primarily with diverse populations, including elderly patients, individuals recovering from surgery or injury, and those managing chronic conditions. Providing physical therapy in a home setting allows you to tailor interventions based on unique patient circumstances, enhancing the effectiveness of rehabilitation and promoting better overall outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Possession of a valid DPT license in the State of Florida.
• Current state licensure as needed for specific service locations.
• Certification in CPR from the American Heart Association for Healthcare Providers is mandatory.

Preferred Experience

• While 2 years of relevant experience are preferred, we welcome applications from new graduates eager to grow and flourish within an established and supportive clinic setting.

Compensation and Benefits
