Key Responsibilities

• Conduct patient evaluations that include a thorough review of patient history, physical assessments, and formulating detailed treatment recommendations.
• Communicate evaluation findings effectively to patients and their families, working together to develop tailored goals that reflect their individual recovery journeys.
• Implement evidence-based treatment protocols derived from patient evaluations, allowing for modifications based on patients' progress and evolving health circumstances.
• Collaborate with other healthcare professionals, documenting any changes in patient conditions that require amendments to treatment plans or specialist referrals.
• Carry out discharge planning and follow-up care, ensuring documentation is precise, timely, and outlines each patient's care journey comprehensively.
• Manage time efficiently to meet productivity expectations, maintain an organized schedule, and uphold punctuality in all professional commitments.
• Engage in community outreach and marketing efforts, helping to increase access to our services and promoting the benefits of physical therapy to potential patients.
• Contribute to achieving departmental goals and objectives while adhering to compliance and regulatory standards.

Patient Population / Client Focus

Your primary focus will be working with diverse patients requiring personalized rehabilitation services within their homes. This may include individuals recovering from surgery, those with chronic pain conditions, or patients requiring support for mobility issues. Your role is critical in helping these individuals regain functionality and independence, emphasizing the importance of tailored treatment approaches.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Valid DPT license to practice in the State of Florida.
• Current American Heart Association BLS for Healthcare Providers CPR certification.
• Commitment to ongoing education and professional growth.
• Effective communication skills and the ability to work collaboratively within a multidisciplinary team.

Preferred Experience

While we require at least two years of hands-on clinical experience, applicants who are new graduates or have relevant educational background and exceptional skills are encouraged to apply. Prior experience in home care settings is beneficial but not mandatory.

Compensation and Benefits

The remuneration for this full-time position ranges from $100,000 to $120,000 annually, complemented by a comprehensive benefits package.
