Work Environment

This role is based in a home care setting, enabling the Physical Therapist to work directly with patients in their own environments. The typical work hours are Monday to Friday from 9 AM to 5 PM, allowing for a balanced work schedule. The therapist will travel to patients' homes within the West Palm Beach area, fostering a unique opportunity to build strong, personal relationships with clients while providing convenient and effective treatment.

Key Responsibilities

• Conduct initial assessments to evaluate patients' physical conditions and needs.
• Develop and implement individualized care plans, utilizing evidence-based practices to promote recovery.
• Provide hands-on therapy and interventions, employing various treatment modalities suited to patients' specific challenges.
• Maintain an appropriate caseload to ensure quality care and patient focus.
• Regularly monitor and document patient progress, adjusting treatment strategies as necessary to achieve desired outcomes.
• Educate patients and their families on exercises and home care techniques to support ongoing rehabilitation.
• Collaborate effectively with other healthcare professionals to optimize patient care and treatment coordination.

Patient Population / Client Focus

The Physical Therapist will primarily work with a diverse group of patients requiring therapy for orthopedic conditions, post-surgical rehabilitation, and general musculoskeletal issues. The patient demographic will include individuals of all ages who need personal, tailored therapy solutions that allow them to regain independence and function in their daily lives. As the therapist directly impacts the recovery process, your role is crucial in encouraging patients through their rehabilitation journeys.

Qualifications

• A Bachelor's Degree in Physical Therapy or higher from an accredited program.
• Active Physical Therapist license in Florida with no history of sanctions or board proceedings.
• Strong ability to implement a wide range of manual therapy techniques and a comprehensive understanding of therapeutic exercises.
• Proficiency with Electronic Medical Record (EMR) systems, especially WebPT, for efficient documentation and patient management.

• Proven track record of developing and executing successful treatment plans and exercise programs.

Compensation and Benefits
