Work Environment\n

As a part of a Community Health Clinic framework, you will primarily engage with families through telehealth methodologies, ensuring a bridge between clinical expertise and accessible, compassionate care.

Key Responsibilities\n

• Conduct thorough evaluations and assessments to identify developmental needs of children aged 0-3.

• Collaborate in the development and implementation of Individualized Family Service Plans (IFSPs) tailored to each child and family's unique circumstances.

• Monitor and assess progress, actively participating as a key member of the interdisciplinary team.
\n
• Deliver essential support and resources to families, equipping them with strategies to enhance their child's developmental milestones.

• Adhere to all applicable Federal and State regulations and standards to ensure safety and compliance.

• Develop strong, trusting relationships with children and families, creating an environment conducive to learning and growth.

• Utilize evidence-based practices to inform recommendations while adapting to various learning styles of children.

• Implement strategies designed to promote development in natural settings, utilizing coaching models for effective teaching.

• Regularly communicate with families regarding developmental progress and advised interventions, priorities, and services required.

• Consistently maintain high standards of professionalism and ethical practices in all interactions.

Patient Population / Client Focus\n

This position directly supports the youngest and most vulnerable members of our community—children from birth to three years old who may be experiencing developmental delays or disabilities. Qualifications\n

• Master's Degree in Social Work or a Doctorate in Social Work.

• Current licensure as a Licensed Clinical Social Worker (LCSW) with the Department of Public Health.

• Minimum of one year of experience working with children in early intervention settings.

• Valid driver's license to travel as needed for family visits or community events.
\n
• Ability to effectively communicate and build rapport with children and families while demonstrating empathy and respect.

Compensation and Benefits\n
