Work Environment

This position is focused on home care, where you will provide therapy inside patients' homes, creating an intimate and personalized therapy environment. Key Responsibilities

• Conduct detailed evaluations of patients in their home settings, assessing their physical abilities and challenges while documenting findings accurately.
• Design and implement effective treatment plans using a variety of methodologies, including therapeutic exercises, hands-on techniques, and modalities aimed at pain relief.
• Monitor patient progress closely, adjusting treatment goals and techniques as necessary to optimize recovery outcomes.
• Provide education and training to patients, their families, and caregivers on proper techniques for exercises, assistive devices, and general wellness strategies.
• Maintain thorough documentation of patient interactions and responses to treatment, ensuring compliance with healthcare regulations and quality standards.
• Participate in multidisciplinary team discussions to align on patient care approaches and strategize on the best methods to support recovery.
• Stay abreast of advancements in the field of physical therapy through continuing education and collaboration with professional organizations.

Patient Population / Client Focus

The primary focus of this role will be on patients recovering from injuries, illnesses, or surgeries, requiring personalized rehabilitation plans that cater to their specific needs and conditions. Our diverse patient population ranges from pediatric to geriatric clients, each presenting unique challenges and potential for growth. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Active and valid State License in Physical Therapy.
• Strong verbal and written communication skills, fostering effective patient relationships and collaboration with caregivers and other healthcare professionals.
• Proficient in using electronic medical record systems and word processing software.
• Adaptable and able to thrive in various home environments, displaying agility in approach and treatment plans.

Preferred Experience

• A minimum of 2 years of experience as a licensed Physical Therapist is preferred but not mandatory; recent graduates are encouraged to apply.
