Key Responsibilities\n

• Conduct comprehensive assessments and develop individualized treatment plans that address the specific needs of your clients.

• Regularly communicate with supervisors, direct care staff, and family members to discuss each patient's progress and any necessary adjustments to the care plan.

• Provide a tentative schedule of appointments, home visits, and observations to ensure consistent monitoring of client advances.
\n
• Coordinate necessary home visits to conduct in-home assessments and observations, ensuring a holistic view of a client’s living situation.
\n
• Implement the established program plans effectively and ensure that all actions comply with legal and ethical standards.
\n
• Participate actively in Interdisciplinary Team Meetings to foster collaboration among various care providers.
\n
• Complete and maintain timely monthly and quarterly progress notes, accurately reflecting client development and needs.
\n
• Organize and deliver training sessions to certify that all staff is competent in program delivery and documentation.
\n
• Conduct follow-up interviews and home visits to ensure ongoing support for clients and to adjust care approaches as needed.

• Act as a liaison between the organization and the wider mental health community to foster supportive relationships for both your clients and development goals.

• Contribute to developing training materials and procedures to enhance program efficiency and efficacy.

Qualifications\n

• A solid understanding of and experience in family therapy, couples therapy, trauma-informed care, or substance misuse counseling is preferred.
