Key Responsibilities\n

As a Licensed Professional Counselor at Recovery Wellbeing Counseling Center, your day-to-day responsibilities include:

• Conducting thorough evaluations and assessments to identify and address the mental health needs of patients receiving care.

• Facilitating communication among supervisors, direct care staff, family members, and guardians to provide regular updates on patient progress.

• Developing and sharing treatment plans with supervisors, including scheduling appointments, home visits, and follow-up observations to ensure continuity of care.

• Arranging and conducting home visits for observational assessments and patient check-ins as needed, ensuring that care strategies are implemented accurately.

• Maintaining comprehensive records and documentation to ensure seamless information access for all relevant parties.

• Participating in interdisciplinary team meetings, providing insights and feedback to enhance collaborative approaches to client care.

• Completing monthly and quarterly progress reports to track patient outcomes and program efficacy.

Qualifications\n
