Key Responsibilities\n

• Conduct thorough patient evaluations to assess needs and develop tailored treatment plans.

• Facilitate effective communication between supervisors, direct care staff, family members, and other stakeholders to keep everyone informed about patient progress.

• Document a comprehensive plan that outlines scheduled appointments, home visits, and observations, ensuring accountability in service delivery.

• Organize and execute home visits for assessments, chart reviews, and collecting data to support patient care initiatives.

• Ensure unwavering implementation of program strategies to provide high-quality care.

• Participate in Interdisciplinary Team Meetings and contribute to collaborative discussions to enhance care quality.

• Prepare and submit monthly and quarterly progress reports that reflect patient outcomes and program effectiveness.

• Facilitate training sessions for staff to ensure clarity in program execution and documentation practices.
\n
• Continuously follow up through interviews and observations to adapt care plans to evolving patient needs.

• Maintain accurate and organized records, making vital information accessible to authorized personnel.

• Act as a liaison between the local community and the mental health sector to help clients achieve their development goals.

Patient Population / Client Focus\n

This position focuses on a diverse patient group, including children, adolescents, and adults dealing with various mental health concerns such as anxiety, depression, trauma, and substance use disorders. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Oregon.

• A minimum of two years of practical counseling experience, ideally within a clinical setting.

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance use recovery is strongly preferred.
