Key Responsibilities\n

• Conduct comprehensive ongoing assessments to identify and address the unique needs of each patient, ensuring tailored interventions are implemented effectively.

• Facilitate clear and timely communication regarding an individual’s progress among supervisors, direct care staff, family members, and other involved parties.
\n
• Prepare and share a tentative schedule of appointments, home visits, and observations with supervisors to monitor the advancement of work goals.
\n
• Conduct home visits as required, allowing for in-depth assessments through observation, chart reviews, and staff interviews.
\n
• Monitor and ensure the effective implementation of approved program plans while actively participating in Interdisciplinary Team Meetings and other relevant discussions.
\n
• Complete detailed monthly and quarterly progress notes to document client advancements, challenges, and strategic adjustments.
\n
• Organize and conduct training sessions for staff and team members to ensure competency in the program’s plan and documentation practices.
\n
• Provide ongoing support through follow-up interviews, home visits, and observations tailored to the evolving needs of your clients.
\n
• Maintain organized and accurate records to guarantee comprehensive information availability for all relevant parties.
\n
• Act as a liaison between the public and the mental health community, leveraging relationships to create and foster individual development goals.
\n
• Assist in refining the procedures for training materials aimed at enhancing program efficacy.

This role primarily serves a diverse patient population, including adults and youth experiencing a range of challenges such as trauma, substance misuse, and interpersonal relationship concerns. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required for this position.

• Possession of an active and unrestricted Professional Counselor (LPC) license in the state of Georgia is essential.

• Specialization in areas such as family therapy, couples therapy, trauma, or substance misuse is highly desirable.
