Key Responsibilities\n

• Conduct comprehensive assessments to identify the individual needs of patients, ensuring a personalized treatment plan is established.

• Maintain active communication with supervisors, peer counselors, family members, and other healthcare professionals regarding patient progress and treatment adjustments.

• Prepare and provide a schedule of patient appointments, home visits, and ongoing evaluations to supervisors, ensuring clarity in work expectations.

• Facilitate home visits for in-home therapy sessions, maintaining a focus on building rapport and facilitating an effective therapeutic environment.

• Monitor the implementation of treatment plans, adjusting goals in response to patient needs while ensuring high standards of care are maintained.

• Participate in interdisciplinary team meetings to collaborate on patient care strategies and enhance service delivery.
\n
• Compile monthly and quarterly progress notes to evaluate treatment effectiveness, using both quantitative and qualitative metrics.

• Organize and lead training sessions for staff, ensuring all team members are proficient in implementing treatment plans and documentation practices.

• Provide follow-up care through interviews and home visits, proactively addressing any ongoing patient concerns or questions.

• Act as a liaison between the organization and the wider mental health community to advocate for patients and enhance care options.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a relevant field is required.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
