Work Environment

This Physical Therapist role is based in a home care setting, providing direct therapy services to residents in their own environments. Key Responsibilities

• Conduct thorough evaluations of residents' physical capabilities to determine their specific needs and establish a baseline for treatment.
• Design and implement customized treatment plans to address the unique challenges faced by each resident, focusing on enhancing strength, mobility, and functionality.
• Utilize a variety of therapeutic techniques, including therapeutic exercises, manual therapy, and state-of-the-art modalities, to effectively treat physical impairments and disabilities.
• Regularly track and document residents' progress, using assessments to adapt treatment plans and ensure they meet evolving needs.
• Collaborate with an interdisciplinary team, including physicians, nurses, and occupational therapists, to deliver integrated care that supports broader health objectives.
• Educate residents and their families, providing them with home exercise programs and strategies for maintaining physical health after therapy concludes.
• Stay current with all relevant regulations and professional standards to guarantee compliance and uphold the highest levels of professional integrity.

Patient Population / Client Focus

In this role, you will primarily work with residents of varying ages who may experience mobility challenges due to injury, surgical recovery, chronic conditions, or age-related impairments. Understanding the unique needs of geriatric patients is essential, as many of our residents require specialized approaches to achieve the best outcomes in their day-to-day lives.

Qualifications

• Current licensure in Physical Therapy in the state of New York.
• Education with a Bachelor's or higher degree, such as a Master's or Doctorate in Physical Therapy from an accredited institution.
• Strong communication, organization, and interpersonal skills, enabling effective engagement with patients and colleagues.

Preferred Experience
