Key Responsibilities\n

As a Licensed Professional Counselor at Reliant Health Center, you will be responsible for:

• Conducting comprehensive assessments to evaluate client needs and devise tailored treatment plans.
\n
• Facilitating communication amongst supervisors, care teams, families, guardians, and other support systems regarding client progress and treatment effectiveness.
\n
• Developing and maintaining tentative schedules for appointments, home visits, and other interventions, ensuring thorough documentation of treatment advancements.
\n
• Coordinating home visits and virtual consultations to support in-home observations, chart reviews, and effective data analysis for optimal client outcomes.
\n
• Implementing and monitoring treatment strategies to ensure the program plan is executed effectively.
\n
• Participating in Interdisciplinary Team Meetings and relevant collaboration sessions to discuss patient care and organizational developments.
\n
• Documenting monthly and quarterly progress notes, ensuring a clear record of client engagement and treatment effectiveness.
\n
• Providing training and guidance to staff to ensure competency in implementation of treatment plans and related documentation.
\n
• Executing follow-up interviews and ongoing assessments to adapt services to the evolving needs of clients.
\n
• Acting as a liaison between clients, families, and the mental health community to further support the achievement of developmental goals.

• Contributing to the creation of procedures for training materials aimed at enhancing client programs.

Reliant Health Center offers a competitive salary range of USD 90,000 to 100,000 per year, commensurate with experience and caseload.
