Key Responsibilities\n

• Diagnose and treat a wide range of mental, emotional, and behavioral disorders, making necessary adjustments to care plans based on ongoing assessments of patient status.

• Perform thorough psychosocial assessments to identify patient needs and develop individualized treatment strategies, collaborating with patients and their families to promote effective care.

• Facilitate referrals to various community resources and treatment programs to advance patients’ overall well-being. This may include connecting them with psychiatrists, support groups, or specialized services.

• Work alongside a multidisciplinary team to collaboratively create discharge plans, ensuring continuity of care as patients transition out of direct services.

• Conduct ongoing evaluations of new, current, and discharged patients, prioritizing their needs and organizing caseloads for optimal service delivery.

• Provide critical crisis intervention and bereavement support, implementing social work strategies that respond effectively to the dynamic challenges faced by patients and their families.

• Educate and coordinate with various patient care teams and external agencies to expedite quality care and address any barriers that may hinder timely service delivery.

• Serve as an advocate for patients and their families, ensuring open lines of communication with all stakeholders involved in their care.

Patient Population / Client Focus\n

The licensed counselors will primarily serve individuals within the community health space, addressing a diverse array of mental health issues. This includes working with clients experiencing anxiety, depression, trauma, and other emotional challenges. Qualifications\n

• A Master’s Degree in Social Work or a related field, obtained from an accredited institution.

• Current State Licensure as an LCSW, LMFT, LPC, or Psychologist.

• Demonstrable familiarity with community mental health resources and services.

• Excellent written and verbal communication skills, enabling effective interactions with clients, families, and healthcare teams.

• A strong ability to build rapport and establish positive therapeutic relationships with clients.

Compensation and Benefits\n

We offer a competitive annual salary ranging from USD 90,000 to 100,000 , commensurate with experience and qualifications, along with a sign-on bonus of up to $1,000. Our counselors enjoy a streamlined approach to case management and client note documentation through a user-friendly platform, providing both time and cost savings.
